Add an Evidence of Property Form

  1. At the Home Base, click Clients & Files.
  2. From the dropdown menu above the list, click Customers.
  3. Select the desired customer from the list.
  4. Click the Policy List button to access the Policy List.
  5. From the Policy List, select the appropriate policy, click Options, and then click Evidence of Property.
  6. Select ACORD 27 - Evidence of  Property or ACORD 28 - Evidence of Commercial Property, and click OK.

    Note:
    For personal lines applications, this choice does not apply; the ACORD 27 is used by default.
  7. If an Evidence of Property already exists, it displays on the screen. To add another one, click File, Add.
  8. If an Evidence of Property does not already exist, click OK in response to the question: "Is This a New Evidence of Property?"
  9. The Evidence of Property Defaults screen displays. Highlight each appropriate policy and click Select.  When all selections have been completed, click OK.
  10. The Evidence of Property form displays. Review all information on each tab. Add any additional information necessary under Remarks or Notes.
  11. Click File > Save to save your changes.
  12. To print this form, click File > Print.
  13. Make the appropriate selections on the Print/Communication window.
    1. Select the appropriate Printer from the dropdown list.

      Note: Click Printer Setup and make any necessary changes if you need to amend the settings for the selected printer.
    2. The date defaults to the current system date but can be changed if necessary.
    3. To change the Producer, double click in the field to the right and make the appropriate selection. Delete the contents of the Producer field if you do not want the producer's name to print on the evidence.
    4. To change the Company, double click in the field to the right and make the appropriate selection. Delete the contents of the Company field if you do not want the company's name to print on the evidence.
    5. Change the Agency and/or Branch if necessary.
    6. To add a signature, double click the Signature field to choose from available signature files.
    7. In the Additional Interests frame, determine whether you would like to print the form for All or Selected Additional Interests.
    8. Determine which Form Version you would like to use.
  14. You can opt to Queue the evidence for printing, to Print it immediately, or to Preview it before printing.
  15. Once you have finished filling out the screen, click OK.
  16. If you chose to print the form for Selected Additional Interests, the Additional Interests To Be Queued screen displays. Select the appropriate lienholder and click the right arrow to add this lienholder to the queue. When you have finished modifying the queue, click OK.
  17. You may be prompted to fill out an activity. Fill out the activity detail, click OK, and click Yes to confirm the activity entry.
