Add an Evidence of Property Form
- At the Home Base, click Clients & Files.
- From the dropdown menu above the list, click Customers.
- Select the desired customer from the list.
- Click the Policy List button to access the Policy List.
- From the Policy List, select the appropriate policy, click Options, and then click Evidence of Property.
- Select ACORD 27 - Evidence of Property or ACORD 28 - Evidence of Commercial Property, and click OK.
Note: For personal lines applications, this choice does not apply; the ACORD 27 is used by default.
- If an Evidence of Property already exists, it displays on the screen. To add another one, click File, Add.
- If an Evidence of Property does not already exist, click OK in response to the question: "Is This a New Evidence of Property?"
- The Evidence of Property Defaults screen displays. Highlight each appropriate policy and click Select. When all selections have been completed, click OK.
- The Evidence of Property form displays. Review all information on each tab. Add any additional information necessary under Remarks or Notes.
- Appl Info- Most of the information on this screen defaults from the application. Review it to make sure it is accurate.

- If this evidence is replacing a priorthat evidence, select the appropriate date from the Replace Prior Evidence Dated dropdown.
- Check Continue Until Terminated if the policy is issued on a continuous basis, with no set expiration date.
- Location

- In order to add a location to the form, click the Add button below the button bar.
- Fill out the address and enter a Description. Select the appropriate radio button to determine whether this is a Building or Business Personal Property.
- To add another location, click Add.
- Coverage- On the Coverage 1 and Coverage 2 tabs, select Yes, No, or N/A for each type of coverage listed. For some coverages, you may need to enter a limit and/or deductible.
- Remarks - Be sure to add the number of days for Cancellation in the appropriate field.
- Additional Interest- You cannot print an Evidence of Property unless Additional Interests are listed. If a previous Evidence of Property exists for this policy, all previous Additional Interests default onto the new form. If no Additional Interests are currently included, do the following.

- Click the Add button below the button bar.
- Double click the Lien # field to select from a list of lienholders already in your system.
- To add a lienholder from this screen, enter a new seven-character code and click Add. Fill out the lienholder's information on the Lienholders screen that displays and click OK. The lienholder is now added to your Lienholder List and the information is entered on the form.
- You can also bypass the Code field and enter the lienholder's information directly on the Additional Interest tab if you do not wish to add this lienholder to your system.
- Select any appropriate checkboxes in the Interest frame.
- Fill out the Lender Servicing Agent information, if applicable.
- To add another lienholder, click Add.
- Notes: In order to add information to the Notes tab, you need to click the Add button below the button bar. After you have typed the body of the note, click Add again if you wish to add another note. The notes do not print as part of the form; they print separately.
- Click File > Save to save your changes.
- To print this form, click File > Print.
- Make the appropriate selections on the Print/Communication window.
- Select the appropriate Printer from the dropdown list.
Note: Click Printer Setup and make any necessary changes if you need to amend the settings for the selected printer.
- The date defaults to the current system date but can be changed if necessary.
- To change the Producer, double click in the field to the right and make the appropriate selection. Delete the contents of the Producer field if you do not want the producer's name to print on the evidence.
- To change the Company, double click in the field to the right and make the appropriate selection. Delete the contents of the Company field if you do not want the company's name to print on the evidence.
- Change the Agency and/or Branch if necessary.
- To add a signature, double click the Signature field to choose from available signature files.
- In the Additional Interests frame, determine whether you would like to print the form for All or Selected Additional Interests.
- Determine which Form Version you would like to use.
- You can opt to Queue the evidence for printing, to Print it immediately, or to Preview it before printing.
- Once you have finished filling out the screen, click OK.
- If you chose to print the form for Selected Additional Interests, the Additional Interests To Be Queued screen displays. Select the appropriate lienholder and click the right arrow to add this lienholder to the queue. When you have finished modifying the queue, click OK.
- You may be prompted to fill out an activity. Fill out the activity detail, click OK, and click Yes to confirm the activity entry.
